Dr. Slutsky performs initial evaluations for Work-Related Hearing Loss Claims. The evaluations are at no cost to patients as long as they have worked for an Employer in Washington State who has paid into the L & I State Fund (for the patient), or their Employer is  / was  Self-Insured for Washington State Workers' Compensation Insurance.

*L&I Insurance was optional prior to 1972 .*

Self Insured Claims: The patient must obtain an SIF-2 form from their Employer /  Employer's T.P.A. prior to their appointment.

Patients are eligible to file Washington State claims for work related hearing loss if they are retired and/or if their Company(ies) are no long in business. This includes owners of business's who have paid for their own L&I insurance.

Please Note: We do not Dispense Hearing Aids. Dr. Slutsky helps patient with the paperwork and performs the (necessary) medical exam to initiate a WA. LNI claim for Hearing Loss. If LNI approves the claim for Hearing Aids, patients go to their Hearing Aid Provider for Hearing Instruments
